Record sheets

Being nosey I'd like to know how people record casualties, morale states, that sort of thing. Being a fastidious sort of chap , I tend not to like scribbling figures on a scrappy piece of paper but prefer something like the following.
General Command ability
Plodder
Average
Good
Inspirational

For some b*****d reason it won't translate from the preview page to the forum but the command ability and unit status columns are supposed to be further to the right, allowing space for names and titles to be inserted and the casualty figures even further over.
You'll have to imagine the whole thing neatly lined and all the items put in boxes, you know what I mean.
THe whole thing to be laminated and irrelevant data deleted with a chinagraph and the whole wiped clean to be used for the next game.
Obviously the numbers are crossed out as the unit loses casualties.
I'd ask you all to bear in mind that I've not actually played this game, so is there anything else needed?
